Dated: 11-01-2022 Quote Request for the Supply, Delivery, Installation and Maintenance of one (1) Tissue Processor R/Sir/Madam I am Dr.Asma Kashif from KK Traders. KK traders is Ontario based, a small and medium-sized Trading, supplier, intending and third party tool manufacturing company, specializing in end-to-end supply chain solution (E2E) services which includes Products requirements and designing, sourcing, import, procurement, logistics, clearing, installations, commissioning, erection, after sale supports, service and all other things. We’re planning to Supply, Delivery, Installation and Maintenance of one (1) Tissue Processor In Ontario Govt via Tender. We would like to request a price quote for the above mentioned product with following list of items and configuration: Background The Ontario Forensic Pathology Service (OFPS), a part of the Ministry, provides forensic pathology services under the Coroners Act and works closely with the Office of the Chief Coroner (OCC) to ensure a coordinated and collaborative approach to death investigations in the public interest. When a coroner requires an autopsy to answer questions about a death, an autopsy is ordered from the OFPS. Of the approximately 19,000 deaths investigated by coroners annually, about 9,600 undergo a medicolegal autopsy performed by pathologists working under the auspices of the OFPS. These autopsies are conducted in Regional Forensic Pathology Units and community hospitals across the province. Histology services are provided by the OFPS Provincial Forensic Pathology Unit (PFPU), which processes over 3,600 tissue specimens per month. Tissue specimens collected through an autopsy are processed in the PFPU’s Histology laboratory and placed on microscope slides for the forensic pathologist to review as part of the autopsy workflow process. The timelines for tissue processing and the generation of microscope slides normally takes several days, depending on workload volume. The histology unit of the PFPU requires a tissue processor to prepare tissue samples for sectioning and microscopic examination. 1.3 Deliverables In accordance with the requirements set out below, the Ministry requires the Vendor to supply, deliver, install and maintain one (1) tissue processor. All Deliverables shall be new. Demonstration models, used, refurbished or prototype instruments will not be accepted by the Ministry. 1.3.1 System Requirements A) Performance Features • Dual or double retort processor • Floor standing tissue processor, dimensions must not exceed 100cm deep and 80 cm wide • Capacity to process up to 300 cassettes per cycle • Must use rectangular baskets with dimensions of 25cm x 29cm and height 6.5cm that can directly be transferred into retort station of embedding station (no circular baskets) • Routine processing cycle may not exceed 15 hours per cycle • Capacity to schedule and load a run up to 4 days in advance • Fume extraction system/filters • Reagent management system with fluid exchange backup (to manage short fills, errors) • Must alert user to any problems that occur (reagents, temperature, pressure) B) Power Requirements • Canadian Standards Association (CSA) approved; and • 120-volt standard power plug. C) User Manuals • One (1) electronic copy (saved on a USB) of all applicable operating, maintenance, service and troubleshooting manuals (in the English language) shall be provided upon delivery of the equipment to the Ministry. 1.3.2 Warranty The Vendor shall provide a minimum one (1) year warranty against manufacturer defects for the Deliverables at no additional cost as described below. The warranty will take effect from the date of acceptance of the tissue processor by the Ministry. During the warranty term and any subsequent extended warranty terms, the Vendor shall fully warrant the equipment and components (including any software) to be free of any defects in materials, workmanship and design deficiencies. If the Ministry or the Vendor identifies any apparent or latent defects or deficiencies in the Deliverables during the initial one-year warranty or any subsequent extended warranty terms if purchased by the Ministry, as set out in section 1.3.3, the Vendor shall repair or replace the defective or deficient equipment or any component thereof at no cost to the Ministry within five (5) Business Days of receiving notification from the Ministry. RFB # OCC-0027 (Tissue Processor) Page 35 of 49 1.3.3 Extended Warranty The Vendor shall provide the Ministry with the option of purchasing up to five one (1) year annual extended warranties for the Deliverables. If purchased by the Ministry, the first one (1) year extended warranty term shall start upon the expiry of the initial one-year warranty provided with the tissue processor. The Ministry may purchase any of the extended warranties at any time prior to the expiry of the then current warranty period for the Deliverables at the applicable price in the Vendor’s Bid. The minimum one (1) year warranty and any subsequent extended warranty terms shall include, without limitation: 1) all parts, repairs, preventative maintenance and other labour and, if applicable, software upgrades and updates for tissue processor; 2) priority response; 3) an unlimited number of service visits per year; 4) one preventative maintenance call per year; 5) travel and labour at no cost; 6) all parts needed for repair and maintenance; and 7) remote support by phone, email or website contact centre. The Vendor shall have qualified technical service personnel, trained and with field experience on the Deliverables and shall be able to respond to a request for technical or trouble-shooting support or service within one (1) Business Day. Where a request for service requires on site services the Vendor shall supply qualified technical service personnel on-site at the Ministry location identified in Section 1.3.4 within two (2) Business Days of such request. Following all work, the Vendor shall submit a report to the Ministry listing all services rendered and provide the Ministry with a documented update on the performance and equipment current conditions, including any noted deficiencies, recommendations or further work that may be required. 1.3.4 Delivery, Installation, Inspection and Acceptance The Deliverables shall be delivered and installed to the Ministry location below within a maximum of sixty (60) Business Days from the time the order is placed. The Vendor shall not charge the Ministry separately for delivery of the Deliverables. All applicable surcharges including freight and delivery charges must be included in all prices quoted in the Pricing Form in the RFB.
