i need quote on heat seal pharmaceutical blister cards

RFQ Description

Need quote that combines two products.-Pharmaceutical Heat Seal Cards-PVC Blister Inserts.Quote must include both products. Target price is .10-.12 USD for both.-initially 50, 000 cards and 50, 000 blisters per month-orders will steadily increase over 1 year to 500, 000 each per month.Cards:-white, food grade, 30 point, SBS virgin paperboard, die cut and perforated-9 inch X 12 inch (folds into 6X9 card)-30 holes die cut exactly like sample (to match blister)-half of card needs to be sealed with pharmaceutical grade laminated aluminum foil-other half of card needs to be coated with food grade adhesive for heat sealing by thermo-press machine.-measurements/dimensions of card must duplicate sample product exactly-minimal printing on one side of paperboard-materials must be equivalent in quality to sample-must seal perfectly with thermopress machine at 320 degree F.Blisters:-Food grade, thermoformed, clear color, PVC, .2 MM thickness-must perfectly match/fit card mentioned above-approximately 5 inch X 6.5 inch PVC sheet (fits inside the folded card and heat sealed using thermopress packing machine)-30 bubbles on sheet (must match/fit inside 30 holes of card)-must perfectly duplicate shape of sample(make mold from sample)To see examples of the products I am trying to imitate, see links below.
